Text and translation:

Et ecce leprosus veniens 
And behold, a leper coming

adorabat eum dicens 
adored him, saying,

Domine si vis potes me mundare 
"Lord, if you will, you can make me clean."

et extendens manum
And extending his hand

tetigit eum Iesus dicens 
Jesus touched him, saying,

volo mundare 
"I will it: be thou made clean."

et confestim mundata est lepra eius
And immediately he was cleansed of his leprosy.
